Position Summary

We are seeking compassionate and dedicated Registered Nurses (RN), Licensed Practical/Vocational Nurses (LPN/LVN), Physical Therapists (PT), Physical Therapist Assistants (PTA), Occupational Therapists (OT), Certified Occupational Therapy Assistants (COTA), Speech-Language Pathologists (SLP), and Medical Social Workers (MSW) to join our growing home health team.

Our clinicians provide care in patients’ homes in accordance with physician orders, Nevada state regulations, and agency policies, with a strong focus on dignity, independence, and clinical excellence.

Key Responsibilities

  • Provide skilled home health services according to physician orders and individualized plans of care
  • Perform discipline-specific assessments and ongoing evaluation of patient progress
  • Deliver care within the scope of practice defined by Nevada licensure and certification requirements
  • Educate patients and families on treatment plans, safety, and disease or condition management
  • Collaborate with physicians, clinical supervisors, and interdisciplinary team members
  • Accurately complete all documentation in compliance with Nevada regulations and Medicare guidelines
  • Report changes in patient condition promptly and participate in care coordination
  • Maintain patient confidentiality and comply with HIPAA regulations
  • Uphold the values of a family-owned organization focused on ethical, patient-centered care

Qualifications

  • Current, active Nevada license in applicable discipline (RN, LPN/LVN, PT, PTA, OT, COTA, SLP, or MSW)
  • Meets all Nevada State Board and Department of Health & Human Services requirements
  • CPR/BLS certification (as applicable)
  • Home health experience preferred, but not required
  • Valid Nevada driver’s license, reliable transportation, and current auto insurance
  • Ability to work independently while remaining connected to a supportive leadership team
  • Strong communication, time-management, and organizational skills
  • Compassionate, reliable, and professional approach to patient care

Physical Requirements

  • Ability to perform essential job functions in a home setting
  • Ability to lift, assist, or transfer patients as required by role
  • Ability to travel to patient homes within assigned service areas

Work Environment

  • Patient care provided in private residences and community-based settings
